Developing comprehensive individualised care plans for people with dementia experiencing responsive behaviours in hospital settings is often complex and requires careful management of a range of clinical, psychosocial and environmental factors. An inter-professional team of specialists in gerontology (geriatrician, pharmacist and nurse) use a series of case studies to explore different complex patient presentations and the development of effective individualised care plans. The clinical complexity of individual presentations to hospital is outlined, alongside theoretical frameworks underpinning care and medication safety.
